Cymbalta is available in delayed release capsule form in 20 mg, 30 mg and 60 mg strengths. The usual starting dose is 30 mg once per day; however, this dose can be titrated by the physician after monitoring initial results and the severity of the disease. Cymbalta capsules should not be opened or chewed, but should be swallowed in the original form. Cymbalta can be taken with or without food and should be stored at room temperature (150C- 300C).
Mechanism of action:
As is the case with other antidepressant drugs, Cymbalta's mechanism of action has not been established; however, the most probable mechanism is related to the increased concentration of neurotransmitters at serotonergic and noradrenergic receptors in the brain. Reports from clinical trials show that Cymbalta has selective serotonin and nor-epinephrine reuptake inhibition (SSNRI) activity at synaptic clefts. The resulting increase in serotonin and nor-epinephrine concentration at synaptic clefts leads to increased serotonin and nor-epinephrine receptor binding activity. Clinical trials have shown that a decreased concentration of neurotransmitters like serotonin in the brain is a main cause of depression and other psychiatric disorders. By increasing the concentration of neurotransmitters, Cymbalta improves neurotransmitter balance in the brain.
Though Cymbalta helps in various central pains, the mechanism of action of pain inhibition has not been established in clinical trials.
Side-effects and safety profile:
The most frequent side-effects of Cymbalta reported in various clinical trials are nausea, constipation, diarrhea, dry mouth, somnolence, sleep problems like insomnia and sleepiness, fatigue, dizziness, withdrawal symptoms, suppressed appetite, and hyper sweating. There are some serious side effects associated with Cymbalta usage like increased suicidal ideation, behavior alteration, mood changes, increased blood pressure, and sexual dysfunctions such as suppressed sexual drive, delay in orgasm and ejaculation. Patients experiencing any of above serious side-effects should consult with a physician to determine the feasibility of continued therapy. Patients withdrawing from Cymbalta therapy should do so gradually under the care and advice of a physician.
Cymbalta, like other antidepressant medication, may lead to suicidal ideation. Patients, both children and adult, should be carefully monitored while taking Cymbalta and any changes in mood should be reported to a physician.
Patients who have recently taken other antidepressant drugs related to monoamine oxidase inhibitor (MAOI) class should not take Cymbalta.
Since Cymbalta may cause hepatotoxicity, patients experiencing hepatic failure, jaundice or other significant liver dysfunction while taking Cymbalta should be properly withdrawn from Cymbalta therapy while under the care of a physician.
Cymbalta is reported to show increased risks of bleeding events.
Cymbalta may cause severe skin reactions like erythema multiforme and Stevens-Johnson syndrome (SJS), so it must be withdrawn at the first sign of rash, blisters, mucosal erosions, etc.
Cymbalta may cause manic or hypomanic conditions.
If patient has a medical history of seizures, Cymbalta must be prescribed with proper monitoring.
Cymbalta may increase fasting blood glucose levels, HbA1c levels, and total cholesterol levels in diabetic peripheral neuropathic pain patients, so it must be prescribed with close monitoring of these factors.
