About Singulair

Generic Name: Montelukast

The active ingredient in Singulair is montelukast sodium, which is classified as a leukotriene inhibitor. Singulair is indicated for various lung disorders such as asthma, allergic rhinitis, etc. Leukotrienes are those chemicals released by the body in response to allergens inhaled when breathing. Leukotrienes cause inflammation of the lungs and constriction of air passages which leads to various asthma-related symptoms. Singulair inhibits binding of such leukotrienes, keeping airways open. Singulair is available by prescription and is highly recommended by physicians due to effective results in a large population.

Singulair works by blocking leukotrienes, which are chemicals produced by the body as an immune response to common allergens. Since leukotrienes contribute to asthma symptoms and cause inflammation within the airway, Singulair makes breathing easier for people with asthma and allergies by keeping the airways open and reducing swelling in the lungs.

Indication/Usage: Singulair is mainly indicated for asthma, exercise-induced broncho-constriction, and allergic rhinitis. In asthmatics, Singulair is prescribed for prophylaxis as well as treatment medication for adults and children 12 months old or older. In cases of exercise-induced broncho-constriction (EIB), Singulair is used as preventative care in patients 15 years old and older. Singulair is widely prescribed in cases of allergic rhinitis for the relief of symptoms related to seasonal allergies. For the treatment of allergic rhinitis, Singulair should only be used for patients 2 years old and older. Patients six months old and older suffering from perennial allergic rhinitis may take Singulair.

Singulair has been proven effective in preventing asthmatic attacks in adults and young children. It is also indicated for allergy-induced runny nose and sneezing in adults and children 6 months old and older.

Dose, Administration and Dosage forms: Singulair is available in film-coated tablets, chewable tablets, and oral granules. Varying strengths of Singulair formulations are easily differentiated by their shape, color and code. Singulair 10mg tablets are film-coated, beige, rounded square-shaped tablets marked with MRK 117 on one side. Singulair 5mg chewable tablets are pink, biconvex and round-shaped with MRK 275 marked on one side. Similarly, Singulair 4mg chewable tablets are also pink in color but are oval-shaped and coded as MRK 711 on one side. Orally used granules come in 4mg strength in white color with a net packing of 500mg in a child-resistant foil pack.

Mechanism of action: Montelukast sodium, the active ingredient of Singulair formulations, is a highly selective leukotriene receptor antagonist. It blocks cysteinyl leukotriene (CysLT1) receptors, preventing air pathways from becoming inflamed and swollen.

Cysteinyl leukotrienes (LT) such as LTC4, LTD4, and LTE4 are produced by the metabolism of arachidonic acid. These LT's are released from various mast cells and eosinophils in the human body and normally bind to cysteinyl leukotriene (CysLT) receptors found in human air pathways. This binding of CysLTs results in constriction of the air pathway by inflaming the airway macrophages and smooth muscles. Singulair relieves such respiratory inflammation to allow free breathing.

Singulair is used to prevent the occurrence of asthma attacks and reduce the symptoms associated with asthma, such as shortness of breath and wheezing. In addition to the treatment of asthma, Singulair can also be used to relieve common allergy symptoms, including sneezing and a runny nose.

Side-effects and safety profile: Singulair has a limited number of side-effects and is normally a safe choice in the treatment of respiratory problems.

Serious side-effects associated with Singulair include agitation, aggression, sleep disturbances, disorientation, hallucinations, self-injury, suicidal thoughts, and shooting pain in various body parts like arms and legs. Other rare but serious issues reported with Singulair include thrombocytopenia, anaphylaxis, hepatic eosinophilic infiltration, palpitations, seizures, epistaxis, cholestatic hepatitis, and arthralgia. Patients experiencing any of the above conditions while taking Singulair should immediately consult their physicians.

Non-serious and easily curable side-effects of Singulair include nausea, vomiting, diarrhea, dyspepsia, body pain, headaches, dizziness, and cough.

Warnings: Always follow instructions as taking more Singulair than prescribed, or taking Singulair for longer than prescribed may result in worsened nasal congestion.

Patients should discuss any history of liver disease with their physicians.
